These forms are for clinical Intake interviews, administered upon intake, by a clinician, in situations where a computer is unavailable or inappropriate during the interview. Examples would be home-interviews, power outages, school interviews, etc. |

Self-completed Intake forms are available for Adolescents and Teachers only. These forms are for situations where a 1:1 clinical Intake interviews, is deemed to be unnecessary or not feasible. Examples include information provided by a teacher when the teacher is unavailable for an interview, or adolescents at the beginning of a group, perhaps in a school setting. |

Paper DISCHARGE, SATISFACTION, AND FOLLOW-UP forms are for collection of Discharge and Follow-up outcome data (6, 12, 24 months).
This is a brief paper form completed during an interview, or self-completed by a parent, teacher or youth at time of discharge, or on follow-up (e.g. 6 months after discharge). It can also mailed to the parent, teacher or youth for them to complete and return.
The forms contains abbreviated versions of the full Intake Interviews, and are restricted to questions re BCFPI's standardized mental health and functioning domains, along with satisfaction items. To enter the data, users create a new BCFPI Parent, Youth or Tecaher form regarding the child, using stage = 'After' or the appropriate follow-up interval (6,12, 24 months).
Data from these forms can be analyzed with current and pending BCFPI reports, comparing pre-post results for single cases, selected cases and groups of cases.

| Database of Evidence-based Interventions (Read-Only Acrobat File) |
The Interview manual describes the background of this database, which is continually extended as new evidence-based interventions become available, It has 3 uses:
- It can be accessed here, for direct examination... what interventions have been shown to be effective for the most common and debilitating children's mental health problems, and what articles support these claims?
- It is the basis for BCFPI's Evidence-based service planning report (See the Interview Manual - "The BCFPI's Evidence-Based Service Planning Report", and the Software Manual - "About 1 Case"). This report summarizes a case's MH and functioning scores, and other relevant characteristics (e.g., demographics, readiness, other concerns, and then details manualized interventions and their citations from this database which have been shown to be effective for similar cases,
- It is the basis for BCFPI's Outcomes Benchmarking report (pending).
